  • Your trust, or the draft we are preparing
  • Recent statements for your bank, investment and retirement accounts
  • The deeds to any real estate you own
  • Your life insurance and annuity policies

About your privacy

This form never asks for account numbers, policy numbers, certificate numbers or Social Security numbers, online or on paper. The name of the institution, the type of asset and a rough value are all we need. We will collect anything else with you directly.

The printed and downloadable versions are the firm's complete forms and do ask for those numbers, because the court and the agencies need them. Write them on the paper copy and bring it with you, or call the office. Please do not send those numbers through this website or by email.
