Have these in front of you

  • The child's birth certificate
  • Names and addresses of the child's parents and closest relatives
  • Details of any money or property coming to the child
  • Three personal references who have known you five years or more

About your privacy

The court will need the child's Social Security number and yours. This form does not ask for either. We take them from you directly when we prepare the petition.

The printed and downloadable versions are the firm's complete forms and do ask for those numbers, because the court and the agencies need them. Write them on the paper copy and bring it with you, or call the office. Please do not send those numbers through this website or by email.
