Have these in front of you
- The death certificate
- The original will and any codicils, if there are any
- Recent statements for bank, investment and retirement accounts
- Deeds, vehicle titles and insurance policies you can find
- Recent bills and credit card statements
About your privacy
This form does not ask for Social Security numbers or full account numbers, even though the court will eventually need some of them. Bring those to your consultation or give them to the office by phone. Put the last four digits in the account fields if it helps you keep them straight.
The printed and downloadable versions are the firm's complete forms and do ask for those numbers, because the court and the agencies need them. Write them on the paper copy and bring it with you, or call the office. Please do not send those numbers through this website or by email.
