- Benefit award letters (SSI, SSD, Medicaid, Medicare)
- Any guardianship court order or letters
- The name and address of the local Social Security office
- Details of where the money is coming from
About your privacy
This form does not ask for Social Security numbers, Medicaid numbers or Medicare claim numbers. We will need them to draft, and we will take them from you directly.
The printed and downloadable versions are the firm's complete forms and do ask for those numbers, because the court and the agencies need them. Write them on the paper copy and bring it with you, or call the office. Please do not send those numbers through this website or by email.
